1. Role Overview & Clinical Scope of Practice

EMTs and Paramedics are the clinical backbone of the pre-hospital 911 Emergency Medical Services (EMS) system. Dispatched to motor vehicle collisions, cardiac arrests, strokes, respiratory emergencies, and acute trauma scenes, they rapidly assess patient stability, control life-threatening hemorrhage, and initiate resuscitation while transporting patients to hospital emergency departments.

Emergency Medical Technicians (EMTs) provide Basic Life Support (BLS)—including CPR, automated external defibrillation (AED), oxygen administration, bag-valve-mask ventilation, spinal immobilization, tourniquet placement, and administration of basic emergency medications (such as epinephrine auto-injectors, naloxone, and oral glucose).

Paramedics represent the highest pre-hospital clinical level, delivering Advanced Life Support (ALS). Operating under physician medical direction protocols, paramedics interpret 12-lead EKGs, perform endotracheal intubation and needle chest decompression, establish intravenous (IV) and intraosseous (IO) vascular access, administer 40+ emergency cardiac and analgesic medications, and perform synchronized cardioversion and transcutaneous pacing.

2. Core Clinical & Technical Responsibilities

In daily clinical practice, a EMT and Paramedic performs the following core responsibilities:

• Respond to 911 emergency dispatches, assess scene safety, and perform rapid primary and secondary trauma/medical assessments.
• Manage patient airways using supraglottic airways, endotracheal intubation, capnography, and mechanical transport ventilators (Paramedic scope).
• Acquire and interpret 12-lead EKGs to identify ST-elevation myocardial infarction (STEMI) and transmit telemetry data to receiving hospital cath labs.
• Establish peripheral IV and intraosseous (IO) lines to administer resuscitation fluids, vasopressors, antiarrhythmics, and narcotics per standing orders.
• Stabilize fractures, control severe bleeding with hemostatic dressings and tourniquets, and extricate patients safely onto ambulance stretchers.
• Give structured clinical handoff reports to emergency department trauma teams and complete NEMSIS-compliant electronic patient care reports (ePCR).

3. Education Requirements & Accredited Schools

Entry-level EMT training requires a high school diploma, CPR certification, and completion of a state-approved 150- to 190-hour EMT course at a community college, EMS academy, or technical institute, including ambulance and emergency department ride-along shifts.

Advancing to Paramedic requires completing a 1,200- to 1,800-hour program (1 to 2 years, often conferring an Associate of Applied Science degree) accredited by CAAHEP upon recommendation of CoAEMSP. Paramedic education includes intensive didactic training in pharmacology, cardiology, trauma management, and 500+ hours of hospital clinical rotations (ER, ICU, OR/anesthesia, OB) and field ambulance preceptorships.

4. National Certification Exams & State Licensure

All 50 states require EMTs and Paramedics to be licensed by the State Office of Emergency Medical Services. Nearly every state utilizes cognitive and psychomotor examinations administered by the National Registry of Emergency Medical Technicians (NREMT) for initial certification at the EMR, EMT, AEMT, and Paramedic (NRP) levels.

Paramedics also maintain active certifications in Advanced Cardiovascular Life Support (ACLS), Pediatric Advanced Life Support (PALS), and Prehospital Trauma Life Support (PHTLS) or ITLS. Recertification occurs every two years via 40 to 60 hours of continuing education.
