1. Role Overview & Clinical Scope of Practice

Inside the hospital operating room, the sterile team directly at the surgical table consists of the Attending Surgeon, the Surgical First Assistant, and the Surgical Technologist (often called the Scrub Tech). Before the patient enters the suite, the surgical technologist "scrubs in," dons sterile gown and gloves, and sets up hundreds of specialized surgical instruments, sutures, retractors, power drills, laparoscopic cameras, and Da Vinci robotic arms on the sterile back table and Mayo stand.

Once the incision is made, the surgical technologist anticipates every step of the surgeon’s anatomy and procedure—passing scalpels, hemostats, electrocautery pencils, vascular clamps, and prosthetic implants smoothly into the surgeon’s hand without the surgeon ever needing to look away from the operative site.

Equally critical, the surgical technologist and the circulating Registered Nurse perform mandatory audible surgical counts of all sponges, needles, sharps, and instruments before incision and before wound closure to prevent retained surgical items and protect sterile aseptic technique throughout the operation.

2. Core Clinical & Technical Responsibilities

In daily clinical practice, a Surgical Technologist performs the following core responsibilities:

• Prepare the operating room prior to surgery by pulling surgeon preference cards and opening sterile instrument sets, sutures, and surgical supplies.
• Perform surgical hand antisepsis (scrubbing), gown and glove self and surgeons, and assist in applying sterile surgical drapes to the patient.
• Pass surgical instruments, retractors, sutures, sponges, and robotic/laparoscopic devices to the surgeon and first assistant during operations.
• Perform strict baseline, intra-operative, and closing surgical counts of all sponges, sharps, and instruments alongside the circulating RN.
• Hold retractors, suction blood and smoke plume from the operative site, apply electrocautery or surgical staplers as directed, and prepare sterile tissue/biopsy specimens for pathology.
• Apply sterile post-operative wound dressings and transport contaminated instrument trays for central sterile decontamination.

3. Education Requirements & Accredited Schools

Aspiring surgical technologists complete a 12- to 24-month Diploma or Associate of Applied Science (AAS) degree accredited by the Commission on Accreditation of Allied Health Education Programs (CAAHEP—via the Accreditation Review Council on Education in Surgical Technology and Surgical Assisting, ARC/STSA) or the Accrediting Bureau of Health Education Schools (ABHES).

The curriculum covers surgical anatomy and pathophysiology, microbiology and aseptic technique, surgical pharmacology, sterilization principles, and step-by-step instrumentation across general, orthopedic, OB/GYN, neuro, cardiothoracic, vascular, and robotic surgery. Students must scrub in on a minimum of 120 documented live surgical cases across core and specialty categories during hospital OR clinical rotations.

4. National Certification Exams & State Licensure

More than 15 states (including New Jersey, New York, Texas, Tennessee, Virginia, Massachusetts, Indiana, Oregon, and South Carolina) have enacted statutory laws requiring surgical technologists hired in hospitals and surgery centers to graduate from a CAAHEP/ABHES-accredited program and hold national certification.

The primary national credential is the Certified Surgical Technologist (CST) awarded by the National Board of Surgical Technology and Surgical Assisting (NBSTSA). Experienced CSTs can advance by completing an ARC/STSA-accredited Surgical Assisting program to become a Certified Surgical First Assistant (CSFA).
