1. Role Overview & Clinical Scope of Practice

Physical Therapist Assistants (PTAs) play a hands-on clinical role in helping patients recover strength, mobility, and independence after orthopedic surgeries, sports injuries, strokes, or debilitating illnesses. Once a licensed Physical Therapist (DPT) completes the initial patient evaluation and establishes the Plan of Care, the PTA carries out subsequent treatment sessions.

During a treatment session, a PTA instructs patients in therapeutic strengthening and stretching exercises, performs joint range-of-motion and soft-tissue mobilization techniques, coaches gait training with crutches or walkers, conducts balance and proprioceptive retraining, and applies therapeutic modalities such as electrical stimulation, ultrasound, cryotherapy, and mechanical traction.

Throughout treatment, PTAs measure objective functional progress (such as goniometric joint angles and manual muscle grades), modify exercise progression within the established Plan of Care, and communicate closely with the supervising PT.

2. Core Clinical & Technical Responsibilities

In daily clinical practice, a Physical Therapist Assistant performs the following core responsibilities:

Deliver skilled physical therapy interventions established in the supervising Physical Therapist’s Plan of Care.
Instruct and progress patients through therapeutic strengthening, core stabilization, neuromuscular re-education, and balance exercises.
Perform gait, stair, and transfer training for post-operative joint replacement, fracture, and neurological rehabilitation patients.
Administer soft-tissue massage, passive stretching, joint range-of-motion exercises, and physical agents (TENS/NMES, ultrasound, heat/cold therapy).
Measure joint angles using goniometers, assess muscle strength, monitor vital signs during exertion, and record daily clinical SOAP notes.
Educate patients and family members on home exercise programs (HEPs), posture mechanics, and fall prevention.

All Physical Therapist Assistants must graduate from a two-year Associate of Applied Science (AAS) degree program accredited by the Commission on Accreditation in Physical Therapy Education (CAPTE).

The five-semester curriculum includes musculoskeletal anatomy, kinesiology and biomechanics, neurorehabilitation, orthopedic pathology, therapeutic exercise, physical agents, and 14 to 16 weeks (550 to 650 hours) of full-time supervised clinical rotations in outpatient and inpatient physical therapy settings.
