1. Role Overview & Clinical Scope of Practice

Clinical, counseling, and neuropsychologists play an irreplaceable role in mental health and neurological diagnostics. While psychiatrists and psychiatric nurse practitioners focus primarily on psychopharmacology, doctoral psychologists are the premier experts in standardized psychometric, diagnostic, and neuropsychological assessment as well as evidence-based psychotherapy.

Using validated diagnostic batteries (such as the WAIS-IV, WISC-V, MMPI-3, ADOS-2, and Halstead-Reitan neuropsychological battery), psychologists evaluate intelligence, executive functioning, memory loss (differentiating Alzheimer’s dementia from depression), traumatic brain injury (TBI), autism spectrum disorder, ADHD, and forensic competency.

In treatment, psychologists deliver gold-standard psychotherapies—including Cognitive Behavioral Therapy (CBT), Dialectical Behavior Therapy (DBT), Prolonged Exposure (PE) and EMDR for PTSD, and Acceptance and Commitment Therapy (ACT). In seven states (New Mexico, Louisiana, Illinois, Iowa, Idaho, Colorado, and Utah) plus the U.S. military/PHS, psychologists who complete a post-doctoral Master of Science in Clinical Psychopharmacology also hold prescriptive authority.

2. Core Clinical & Technical Responsibilities

In daily clinical practice, a Psychologist performs the following core responsibilities:

• Administer, score, and interpret comprehensive psychological, personality, psychoeducational, and neuropsychological test batteries.
• Diagnose complex mental, behavioral, neurodevelopmental, and neurocognitive disorders using DSM-5-TR and ICD-10 criteria.
• Deliver evidence-based individual, group, couples, and family psychotherapy (CBT, DBT, ACT, exposure therapy, psychodynamic therapy).
• Write comprehensive integrative diagnostic evaluation reports with clinical, educational (IEP/504), or forensic recommendations.
• Conduct suicide and violence risk assessments, crisis interventions, and behavioral medicine consultations in inpatient hospital units.
• Supervise psychological trainees, postdoctoral fellows, and master’s-level clinicians, and conduct clinical outcomes research.

3. Education Requirements & Accredited Schools

Independent clinical practice as a licensed psychologist requires a doctoral degree accredited by the American Psychological Association (APA): either a Doctor of Psychology (Psy.D.), which emphasizes direct clinical practice and assessment, or a Doctor of Philosophy (Ph.D.) in Clinical, Counseling, or School Psychology, which balances clinical training with empirical research. (Note: School Psychologists practicing strictly inside K–12 public schools may qualify with a 3-year NASP-approved Specialist in Education [Ed.S.] degree).

Doctoral training spans 5 to 7 years post-bachelor’s, including 3 to 4 years of graduate coursework and supervised practicum placements, a doctoral dissertation, and a mandatory 1-year (2,000-hour) full-time pre-doctoral clinical internship accredited by the APA/APPIC.

4. National Certification Exams & State Licensure

All 50 states and the District of Columbia require practicing psychologists to hold a State Psychologist License. Licensure requires an APA-accredited doctoral degree, 1 to 2 years (3,000 to 4,000 total hours) of supervised pre- and post-doctoral clinical experience, passing the national Examination for Professional Practice in Psychology (EPPP) developed by ASPPB, and passing a state jurisprudence exam.

Psychologists can also achieve specialty board certification through the American Board of Professional Psychology (ABPP) across 15 specialties, such as Clinical Neuropsychology (ABCN), Forensic Psychology, Child & Adolescent Psychology, or Rehabilitation Psychology.
