In inpatient physical rehabilitation hospitals and VA medical centers, recreational therapists work with patients recovering from spinal cord injuries, amputations, strokes, and traumatic brain injuries (TBIs). They coach wheelchair mobility and adaptive sports (such as wheelchair basketball, handcycling, or adaptive kayaking), lead aquatic therapy sessions, and guide real-world community reintegration outings so patients learn to navigate restaurants, grocery stores, and public transit safely before discharge.

In inpatient psychiatric hospitals, substance recovery centers, and memory care units, recreational therapists lead structured group interventions that build stress coping skills, emotional regulation, social interaction, and cognitive stimulation.

2. Core Clinical & Technical Responsibilities

In daily clinical practice, a Recreational Therapist performs the following core responsibilities:

• Conduct standardized APIE (Assessment, Planning, Implementation, Evaluation) recreational therapy assessments of patients’ physical, cognitive, social, and emotional functioning.
• Design individualized therapeutic recreation treatment plans aligned with the interdisciplinary rehabilitation team’s discharge goals.
• Lead adaptive sports, wheelchair mobility skills, aquatic therapy, horticulture therapy, animal-assisted therapy, and expressive arts/music sessions.
• Plan and supervise therapeutic community reintegration outings to help spinal cord injury, stroke, and TBI patients practice real-world community navigation.
• Facilitate coping-skills, anger-management, and leisure-education groups in inpatient psychiatric and substance use treatment facilities.
• Document patient functional progress in the medical record and adapt recreational equipment for physical or sensory impairments.

3. Education Requirements & Accredited Schools

Becoming a recreational therapist requires earning a four-year Bachelor of Science degree in Recreational Therapy or Therapeutic Recreation (or a related major with 18 credit hours of therapeutic recreation coursework and supportive anatomy/psychology courses) from a university program accredited by CAAHEP/CARTE or Council on Accreditation of Parks, Recreation, Tourism and Related Professions (COAPRT).

Students complete coursework in anatomy and physiology, abnormal psychology, human growth and development, assistive technology, and therapeutic recreation assessment, culminating in a mandatory minimum 14-week (560-hour) full-time clinical internship under a CTRS supervisor.

4. National Certification Exams & State Licensure

Healthcare employers nationwide require national certification as a Certified Therapeutic Recreation Specialist (CTRS) administered by the National Council for Therapeutic Recreation Certification (NCTRC). Five states (North Carolina, New Hampshire, Oklahoma, Utah, and New Jersey) also enact state licensure or formal state certification for recreational therapists based on the CTRS credential.

CTRS clinicians renew certification every five years through continuing education hours and professional clinical practice.
